Required Records Birth Certificate (a certified copy) A Baptismal Certificate or hospital record may be used in lieu of a birth certificate provided a certified copy is presented within thirty days Health and Immunization Records Report card/transcript from last school attended Social Security Card Proof of Income, if applying for the National Nutritional School Program Valid Identification of the parent/guardian enrolling the student (Driver’s license, Social Security Card, Passport) Proof of Residence (Utility Bill, Lease) Students who are under 18 years of age and do not reside with a parent must complete a responsible person affidavit. Please visit our district website at: www.episd.org. Click on departments and choose pupil services and required documents and forms will be posted there.

Attendance: If you know your son/daughter will be late or will not be at school, please call the automated attendance line at 915-587-2505. You will need to have the studen'ts name and ID number. Attendance is very important. If a student misses more than 10% of instruction, they will not receive credit for that class.
